New Patient Information
We welcome all new patients! To make your first visit go as smoothly as possible, please read this entire page and follow all instructions.
Before your first visit, please have your former dentist send us a copy of your dental records, including recent radiographs (x-rays.) On your first visit, you will have a dental exam, radiographs (if needed), and a cleaning. Please allow approximately 1.5 hours for this visit.
You will also be asked to fill out a new patient information and insurance form. You can also download the form from our website, print it from your computer, and fill out beforehand to bring with you to your first visit. Simply click on the links on the right hand side of this page to download the appropriate forms. If you need help, contact us.
