We welcome all new patients! Before your first visit, please have your former dentist send us a copy of your dental records, including recent x-rays. On your first visit, you will have a dental exam, x-rays (if needed), and a cleaning. Please allow approximately 1.5 hours for this visit.

You will also be asked to fill out a new patient information and insurance form. You can also download the form here, print it from your computer, and fill out beforehand to bring with you to your first visit. Click on the link below to open the form in a new browser window. Then, click on the printer icon in the toolbar above the form to print.
