Full payment is expected at the time of treatment. Acceptable forms of payment
are cash, check, MC/VISA, Discover, American Express, and CareCredit (www.carecredit.com).
As a courtesyto our patients who have dental insurance we are happy to file your claims and
assist you with benefit issues; however, we remind you that your insurance is
a contract between you and your carrier. Please be prepared to pay an estimated
portion of the charges at the time of your visit.
